Regarding shifting of Address

I am M. Sreedhar Kumar and i have received a message from ICICI Credit Card Division that my card has dispatched through Elbee Courier vide AWB No. 320355070 and sent to my old address at Flat No. 103, II nd Floor, Prime Castle, Ravindra Nagar Colony, Habsiguda, Hyderabad and returned due to shifting. I am working with M/s. Vishnu Chemicals Ltd., at corporate office earlier, due to launch of New project at Vizag i have sent on deputation to Vizag. Now I am Staying at H.No. 26-16-27, China Gantyada, Opp. Lane of E-Seva, Old Gajuwaka, Vizag. So Kindly redirect the card to the above said address. I am ready to produce my photo ID at the time of taking delivery i.e., PAN Card. For Verification purpose i am furnishing my date of Birth as 17.07.1965
