ICICI health saver plan
Not recived policy kit after 7 months

Dear Sir/ madam,

As per your SMS sent to me on Ist june that my policy kit no.11130115/AppNo. HS00572141 is delivered to my adviser Mr. praveen kumar singh ph. 9415512774 gomti nagar branch, lucknow. He promised several times that he will deliver the policy kit but i did't get that kit.

so kindly go through it and take nacessary action againest him, to keep my faith in ICICI intact.
