Unauthorised swip of credit card by icicilombard for the health insurance cover twicely

i amod bothra holding credit card no. 4477466656099008 of icici bank, now the matter is :-from the icici lombard insurance co. ltd had called me on 14.01.08 for the health insurance but i had refused their proposal, even they had swip the amount of apprx.11000 on 18.01.08 and appx. 8000 on 19.01.08 for the same policy twice in the unauthorised way (WITHOUT TAKING MY AUTHORITY TO SWIP THE CREDIT CARD FOR THE SAME POLICY WHICHEVER THEY HAD PROPOSED ME). and when i had received the policy i had complaint to the icicilombard for cancelling the same policies more than 3 times, now they r saying that u had cancelled the policy after 2 month of receiving and i (amod bothra) hav to bear the cancellation charges of rs aprrx. 2500, and they had call me more than 10 times and threaten me to pay the cancellation charges even i had written complaint to them to take legal action against them (icici lombard) and the credit card department who is also involve in the matter.
