I didn't apply for policy they send and blocked the amount in my icici bank account

I would like to cancell my policy no: 4034/FPP/04024260/00/000. BECAUSE I DIDN'T APPLY FOR THIS, (ALREADY I HAVE PARAMOUNT HEALTH SERVICE PVT LTD., POLICY NO 11602144) THE AMOUNT IS BLOCKED IN MY ICICI BANK PREFERRED CREDIT CARD NO: 4477 4771 7048 3008.PLS RELEASE THE AMOUNT, I WON'T USING THE CARD AND THAT CR.CARD WILL BE SURRENDER TO ICICI BANK .TAKE IMMEDIATE ACTION.
