Fudged Signature

Dear Sir... This is related to Health Saver Policy No. 11233384 availed from ICICIPrudential Life.

I was never shown "Electronic Benefit Illustration sheet" in which all the expenses related to policy are mentioned. I gave a written application to Dwarka office regarding this signature forgery. How can I sign a document when it is not shown to me?

Now, I am being informed that I have signed that document. Which is next to impossible.

As I am cheated by this company, would request you to see the matter and arrange my refund. I do not want to associate with a company wherein employees are cheaters and signature is fudged.
