Confirming the credit about the Post dated cheque No. 601342

2. Policy No. 03336998 Life time super Pension scheme.

3. Renewal Premium Receipt No.40262656 dated : 30/6

Dear Sir,

With references to above subject it is very unfortunate that I was to day informed by Rajkot branch that my previous Premium for the period of 30/6 to 31/7 was not credited due to cheque return.

I have submitted PDC No. 601342 dated 21/3drawn on BOB Morbi branch. I was informed that above cheque was bounced. I have visited my bank to know about above mention cheque there I come to know that cheque was not presented for clearing.

So, I request you to credit Rs. One thousand as a Renewal Premium for the period of 30/6 to 31/7 without any delay.

If my policy was laps for any reason mention above should be problem for your well known ins. Company, so do needful in the matter as soon as early.

If anything happen with my above mention cheque number it will be your responsibility
