Imint card not receieved yet

My Imint account number: 9401150598281102.I have not receieved my iMint card till now. I made so many calls to imint customer care for last 3 months but no results. The request numbers are: 380496, 399745. Please help me to get my imint card asap. I am not able to use my reward points.
