Lpg gas connection termination

Sir,

i am having a household lpg gas connection in udaipur, rajasthan on my name.i bear a consumer no.as 7482 and the distributor is aravali gas service, 195, hiran magri sector-11, udaipur (rajasthan)

i bear this gas connection since 12 years with the same distributor.yesterday when i went for booking of gas cylinder, the person at the gas service refused for booking the same saying that a survey was held in the month of february and when asked for showing the documents by survey person, my daughter refused for the same and for that reason they are saying that indane has cut-off my connection.also here i would like to mention that i have lost my original documents regarding the gas connection. so i would request for providing me a duplicate of my documents and also book the gas cylinder else i had to move in the consumer court.
