Unreasonable delay for second lpg cylinde

I have alrdy submitted a complaint with id no.6547/3.but not yet received any rplay, my mothers consumer no is 5354.it is 2002 connection at kunnamangalam indane gas agency, calicut, kerala, .mother have bokked for 2nd cylinder on aug, many times i contact with dealer by direct and telephone, but alas..they are not ready to informe the current status, not only that they are not willing to issue me my D.G.C.Card up to this date, kindly consider this
