Not accepting unique numbe

For claiming the free gift and the air ticket, I filled in the unique number to claim the special offer in the given column along with the membership number and invoice number in the respective columns. I also gave my E-mail ID as required but it is not accepting these numbers and repeatedly asks me to refill the details. It says "INVALID NUMBERS" each time. Please help.
