SV (Subscription Voucher) missing

Dear Sir/Madam,

We have an Indane connection at Hiwarkhed (Rup), Dist:- Akola. The connection is very very old AND the customer No is 000456 where as SV number is 0190556 . The connection is on the name of my Father Shri Rameshchandra Vanjara. Now I want to transfer that connection to Pune. When we approached to the dealer he asked us for the SV (Subscription Voucher). Apparently SV is missing. Actually the connection is so old that we even don't remember that whether SV was given or not.

So I need help in this matter.

What should we do in this?

How do I will be able to get the connection transferred?
