Incorrect document

I would like to state that I have the Kotak policy no. 01051312, client id 51454455. I have to pay my this year's premium of rs 50K. However the document given to me is having incorrect entry at one place. I would like to talk to the officer concerned to rectify the same and deposit the sum. Further, thhe customer service nos are all not working.
