Cancellation of my credit card

With reference to the above, I would like to inform you the following:
1) I have not received my net banking/phone banking/PIN no till date. I have given numerous complaints to customer service but no action till date.
2) I was not receiving regular monthly statements but after firing the customer service department, I am now receiving the same from last 3 months with changed address.
3) I had purchased a TV on 1st January valued at RS. 10, 200/- which was paid by the above card.
Subsequently I have not received the statement nor any alert message from you regarding the pending payment. But I was receiving SMS from you for loans at very attractive terms but not for the payment.
I had complained to your Customer Service many times to reverse the financial/late fee charges and your people advised me that the concerned persons from Loans department would assist me but till date noone has called regarding the same.
Further, they advised me to contact Ashok Nagar Branch for redressal but when I approached them, they said it is regular branch and they cannot view the outstandings and requested me to contact NM Road Office.
Under the circumstances, I would request you to kindly waive the financial/late fee charges and give a zero balance statement otherwise I would no other option but to cancel/surrender my card. I would want to make one thing very clear that I will not make any financial/late fee charges.
I would request you to kindly do the needful at the earliest and confirm per return mail.
