Claimant of Life Insurance Pension

Dear Sir,

On repeated request and letters from the company to forward Micheal Benerdict"s Provident Fund has failed. His Annuity Number is Y08050014. He expired on 5th January. Its been almost 3 years since he has expired. I Marilyn Leene, his wife informed Lawerence and Mayo and submitted his death certificate and attached letter, which they forwarded to Life Insurance Santacruz Mumbai, which till date have not received any response from you all.

Michael Benedict Leene worked in Lawerence Mayo from 1987 - 1999..His d.o.b is 28th March 1962. I Marilyn Leene his wife would like to claim for his provident fund money, his annuity number is Y08030014.

I would be grateful if you can kindly do the needful and reply to this complaint.
