Not Got the Policy Sheet

Dear Sir,

I have taken a policy from your Payyannur branch.

The details as given below,

Date of Commencement: 11/03

Plan & Term: 149/77

Mode of Payment: Qly

I have made three payments for the same policy. But still I have not got the policy the policy sheet which we will get at the starting of policy. the main policy sheet having stamp and seal.

This is the third time I am complaining about the same. After taking action for this complaint, please reply me in the same mail address.

Please do the need ful.otherwise I have to cancel that policy.
