Non payment surrender policy

On 17.12 08, I have surrendered my policy No. 122684199 at branch 126, Daryaganj. It was told cheque will be delivered within 7days. On 30.12 08 I and met Ms Kavita, of PS Dept she told it will be delivered shortly. Today I got a letter dated 20/12 wherein in sl 8 "Pls complete enclosed form duly atteted and witnessed, but no form is enclosed. Can lic tell me when my harassement will be ended?
