Non receipt of Surrender Payment

Sub: Surrender of Jivan Dhara policy bearing no. 440915137.

Dear Sir,

With due respect, I would like to draw your kind attention to the matter that I submitted the original policy bearing no. 440915137 along with form nos. 5074, 3510 and application duly signed and filled along with bank a/c no. for surrender of the same at your branch office in the month of March. But it is very much unfortunate that I have not yet received the payment.

As understood, this delay has been done because of miss-concept of surrender value calculation for Jivan Dhara Policy by your concerned department.

If you are unable to discharge the payment, please furnish me a letter for the same within 5 days or I shall move for strong legal action against the deficit of service.
