Being my policy no:772390510 (122-10) vested andwas instructed to furnish the option form. But I am in need of money and a surrender form is also submitted and application submitted to grant me the surrender value to I.P.P.Cell Zonal office Chennai.

But after a while branche office instructed me to produce a Medical Certificate. It was also done immediately. But since then no more steps were found to be taken in favoure of me by the higher officer. So it is my request to be kind enough to do the needfull without delay.

I have forword a request for sourender along with policy documents to I.P.P Cell, Chennai on 15/10.
