Agent is not giving service

Sir,

This is to bring to your notice that my LIC policy no is 880420448.and my LIC agent name is Kamlakar. Amre. His agent code is 0054491T. He is not giving me any type of service which he promised me at the time of policy. Everytime i have to pay extra money as a late fee. Kindly i request you to remove his agent code and all benifits from my policy. Now i submits the cheques by my self only.
