LOST MASTER FROM YOUR LIC 12G

My LIC Policy no.111159173 with your 12G branch, they lost MASTER POLICY.Since 12/05, no action taken by CRM

office as wellas 12 G branch Nehru Place New Delhi. My D.D.

IS lying there since 21/06 for Rs.15392/-of AXIS BANK.

which was received by Madam Arti on 26/06.No receipt

given to me. DD is with Mr. Pankaj Malik of 12G branch. I can not

under stand what is my fault .kindly help me.
