Wrong issue of policy no. 396139057

I have purchased a policy in the name of miss Ananya my daugther's daughter with payer/proposer myself ravinder kr gupta and paid Rs 6000 by sbi model town ambala city through cheque which was given to miss parveen/ mr aditya agent of max new york life ambala city haryana. they have changed the documents by cuting/correections/tampering in the record and signing my daughter signature to issue policy not required that in the of my daughter shweta goel nominee ananya payer ravinder kr. it has been time and again requested miss parveen/rajeshsud/analjit singh from max new york station i.e karnal/gurgoan, but no response.today mr aditya from max new york life approached/contacted me as per direction of mr puneet and mr jp, to give a affidavet for the correct issue of the policy frm the date of issue of cheque but refussed to give in wriiting the receipt of the affidavet and issue of policy frm the effective date of coverage 11 april.it was brought to the notice of all concern that there is no option left but to put an application before the sp ambala to lodge/register f.i.r against all concern for cheating miss using/miss behaving.
