Agent didnot deposit the premium

Dear Sir,

I am a policy holder of moneyback policy 11 year plan having policy no-269303434 through GTFS under Azamgarh branh UP. I have paid the premium amount to my agent but he didnot deposited the premium for last year. sir how can i extend my policy. please give the status of my policy and remedies to continue my policy.
