Non-Receipt of Statement

NON RECEIPT OF DEMAT ACCOUNT CLOSURE STATEMENTS

ACCOUNT NO. IN300183-13003835, IN300183-13000355 & IN300183-13003844

Sir,

1. We are the holders of the following Mutual Funds, details as under:-

(a) Account No. IN300183-13000355

(b) (i) MORGAN STAN GF UNITS Qty – 200 ISIN INF063A01019

(ii) UTI-MASTERSHARE UNIT SCHEME Qty – 459 ISIN INF189A01038

2. As per the Demat Statement dt 11 May (photocopy attached) the above holdings have been confirmed in my account.

3. The above Demat Account No. IN300183-13000355 was closed by me but to date I have not received the final statements of the closure of the account. I am therefore not aware of the status of the above accounts as the periodic dividends on the above Mutual Funds have not been credited to our bank account. Neither have I received the redemption processed of the above Mutual Funds at para 1 (b) above.

4. As a period of more than one decade has elapsed, you are requested to forward the final statement of closure to me at the earliest at the following address:-
