Purchased at: UniverCell - Visakhapatnam; Invoice No. P.VIS/771 Dt. 12-1

Insurance Premium collected: as per Invoice is Rs. 43.75

My House address is: C/o R.V.B. Krishna Murthy, MIG -69, Autonagar, BHPV Post- Visakhapatnam 530012, My contact Telphone No is Land Line: 0891 - 2589 478.

I have claimed with all the relevent documents to the Insurance Co., through UniverCell-Visakhapatnam, But sorry to inform you that all the documents returned back to my address as it is!

I want my my cell phone Now, What you want me to do Now.
