Surname wrongly mentioned And ID card for policy not received

We Vishambhar Prasad Kamalia & Indra V Kamalia are both the policy holders of your company vide our Order number:20060207045 dated-01/03 to 28/02. We are hereby requesting you to rectify the spelling of our surname, it should be Indra V Kamalia and not Indra V Kamha. From beginning of the policy till date the same has been communicated but the needful has not been done till date.

And also we have not received Identity Cards for our policy till date the same has been reminded to you repeatedly.

Kindly take immediate action for rectifing the spelling and also issue our ID cards on our new address as soon as possible.
