Non-receipt of PAN card

I applied for a duplicate PAN card on December 3, at HDFC Branch, Churchgate (next to K C college). This was because though I had checked and found out my PAN number years ago, when the system was introduced, the actual card never reached me. Though my application (CSF No 703362057-022404259) was given with all supportings in order, I have still not received the duplicate PAN card, nor any communication from the Bank. They had said it would reach me in 4/5 weeks time. It is already more than 3 months.
