Wrong Debit of Amount from ATM

I shreejit is ICICI customer a/c No 007801025229, On 27 Sep 2008 i tried to withdraw Rs500 twice using my ICICI debit card at PNB ATM NAINI, Allahabad (ATM ID D1036900), but the transaction was terminated and cash was not deilvered, How ever the amount was debited from my account.
