Re isssue of my pf cheque

With reference to letter no. 14/28418239/19961/7873 dated 16-02, w.r.t

PF- Rs 16362/- chq no 719322 Dt-31-7 and EPS of Rs 6630/- chq no.252218 Dt 31-07

With reference to the above subject cited, I would like to inform you that, I have received a letter dated 16/02 along with 2 sets of enclosed AsR (blank Sheets) from you saying that my PF cheques have been returned back to your office since my previous account with Citibank got closed, so in order to get them I need to fill those blank ASR sheets (both the copies) and get it authorized from my banker and send it to your office, so that in turn you can re-issue those cheques to my new SBI account. The very same movement itself I have sent you both the copies of ASR sheets dully filled along with required details through speed post dated 18-02, reference No. EK389588040IN but up to know there has been a gap in the feed back, I even tried to call at the mentioned number but I could not get it.

Sir please help me at the earliest because I am in very bad shape financially, for the reason only I had given a request for withdrawal of my funds, sir its my humble request to you is to please re-issue my cheques at the shortest possible time.

its been almost 7 to 8 months following with this, i have tryed calling to the concerned department but did not get proper response, enev on sundays i tried the phones were busy,
