Non reception of policy document

Sir, im serving in army, from Aug monthly RS 375 is deduct from my payment and it goes to PLI but till now i have no document. Policy no shown on my statement of account is 696813, sir i dont know this is my policy or not.please send my document (policy paper), otherwise take some action to stop this deduction and deducted money back. If this policy no belongs to me pls send policy document to the following address.
