Non Receipt of Any bond

I started PLI scheme from STS BSF BANGALORE, yelhanka through APS during year 2006 for a monthly subscription of Rs. 415/-. The amount is continously deducted from my salary every month but i Don't have any bond or any docs regarding this PLI. My PLI No is 000000000808611 (000000000808611). Request intimate from where i will get the concerned docs regarding PLI for future correspondance. My permanant address is as under
