Withdrawal of pf money

Respected sir/madam,

from last 6 (six) months I am asking my ex-employer (hard castle restaurant pvt. ltd) regarding the submission of my provident fund form & they (abdul sapatwala) is telling me that he has submitted my pf form (MH/41618/5745) first in october & then on 06/11/09 & I had inquired in the bandra PF office & they had told me that my form has still not been submitted. so please tell me what shall I do?
