NON ISSUE OF PASSPORT

Dear Sir,

1. I am serving in ARMY in the rank of Major and my name is MD Krishna kumar, presently serving in North East. When I was serving at visakapatnam, I applied passport for my wife. Cash receipt No. is A006630 dt 06 Apr and Key no is 6500663010.

2. I personally approached and corresponded several time but no action has been taken up to issue of the same. In the mean time Since I shifted my family to chennai since I transferred, again approached Visakapatnam office alongwith personal particular form and all information through letter no.4732/pers/01/E1 dated 14 Jan.
