PF Withdrawal Application

Sir,

I have worked in Oursource partners International's Kochin branch. But the company opened the PF account in Karnataka state since their registered office is in Karnataka. I resigned my job on 15th January and submitted my application for withdrawing my PF on April 08. After 5 months i make an enquiry and the company responded that they didn't forwarded my application, since i have not attached a cancelled cheque lief along with the application. I sent a cancelled cheque lief along with the covering letter to the company on last november for further proceedings. But till now i didn't get any reply from the company side or from the PF office regarding this matter. Please do the needful.
