Non receipt of army pli policy

Sir

my name is arun kumar.at the time of policy my rank was EMPI, personal number 136577B.I took the policy in ins valsura army postal office, jamnagar gujarat during june for Rs 100000.every month Rs 395 is being deducted and it is highlighted in my pay statement.but i have no policy number or bond.so please if u could send me the same on arunarun_90@yahoo.co.in. and oblige.
