Non receipt of policy bond

Hi sir this is Prashant Srivastav from Bidar, i had opened my PLI in aug -sep 06 in 405 AIR FORCE STATION BELGAUM.till now i've not received any receipt and/or policy bond. A monthly premium of INR 630 is deducted from my salary.so it is requestd to send my documents as soon as possible.
