Commitment not fulfiled

I had taken country club membership on 8 Dec 08. membership type (kool).mem no MHKPR-315-P . While offering the membership a letter was issued of the offerings (attached). but while accaually when i saw my profile on country club site .nothing was updated as per the offer. when contacted to the main office the branch manager was not in position to fulfill the commitment made in the letter .after lots of follow up & mails there is no response from country club off.
