Misuse of id document provided for taking new sim

I had taken a sim from smart electronics located at aidc guwahati on about 20 june.the sim was activated but the number was terminated on 2nd july.contacting reliance care they told me that because of document not submitted the number was terminated.contacting the store keeper he gave me the no of the retailer.the number is 9707019236.when i gave him complain about he told me that he will check it.but after that he is not answering the call.i have sumitted all the document and it may be misused by the retailer..so i pray you to take necessary steps to help me with the same.

My no was 919508963648.the document provided was on the name of Biren Sarmah h no-1 bylane-1.beltola chariali guwahati-781029.

Plz help me with proper steps to avoid misuse of my document.
