P.F amount not credited in my a/c

Dear Concern,

I have submitted my P.F (P.F a/c no-MH/34020/012527) withdrawal form in the first week of August... through BAJAJ ALLIANZ LIFE INSURANCE CO.LTD, LUCKNOW (U.P). But till date the P.F amount has not been credited to my a/c, as mentioned bellow... Nor any information pertaining to the status of my pf withdraw inspite of being so many reminders to my insurance company and the numbers which had been provided by my ex company of the EPF Office Mumbai are never being picked up so kindly look into the mattter and let me know the status of my PF withdrawl.
