Renewal Note Not issued after taking payment

Policy No: OG-09-1904-1801-00004218 (Expiry on 25-Sept) Renewed using my Credit Card. Payment Rs. 5412/- is recorded in my Card Statement of October. Transaction ID (online payment) - 1376762 dated 14 Sept.

Since then, I am trying to get the Insurance Renewal Note without any success. Online complaints, Feedbacks, contacts on Email with Pune office - all led to NO CLOSURE.
