Insurance policy got confirmed and got cancelled. Both without any intimation

I have subscribed to BALIC plan in May. My Application No is “5400014042”.As part of this plan 1500 is getting debited from my account monthly. I need some clarifications on this.

1.On 28 th, without any intimaition this got cancelled and amount got credited back. I am not aware of this until I get to see the transaction in my e-statement.

2.As per this plan, I got 3 cashless hospitalization cards (for me, my mother and my father) at YAshoda Hospitals and some other hosp. But the problem is for all the 3 cards, the valid till date/the expiry date is mentioned as 12 th jun. I don’t find any sense in that. I just took it a month before and I get these benefits valid for only a month long.

3.I want to know exact details of, under what plan I am covered? what are the charges included in this? And what are the benefits of this plan?

Please clarify me in all the above aspects.
