Not received renewed policy

Dear sir,

I am a health insurance holder of Bajaj Allianz health insurence service. This is my details.

Policy No. OG-11-2401-8409-00000619

As per your renewal notice dated on 04/08. I have paid the amount through cheque no. 249097 dated 11/08 (central bank of india) through agent Joydev Patowari (mob no. 8296280198).

But i still not received any document like Health Card and Policy from you.

I will be kind enough if you send my renewed Policy and Health Card as soon as possible in my address.
