Reject of claim

Dear Sir

Ref to claim no-5769564, As per policy :121400/34/10/87/00000347, Emp code-61804, it has been rejected by you.the Reason is " The same could have been done on OPD basis", It surprised that we are not the right person to decide what to do and how to do treatment.it is a matter of life, so to be decided by Specialist Doctors. So doctor has decided and given the treatment.and accordingly claim submitted by me. You have rejected the claim on perception which is not a justice to your client. You should go by facts and evidence with you. We are not intended to get income from you.it is our rights to get service from you.

steps followed before eye-treatment.

1. Esselmining doctroc treat the patient in our company.

2.refer to eye specialist.

3.eye specialist contacted at Rourkela.

4.Eye specialist immediately reffer to LVPL eye institute BBSR other wise left eye would lost his vision.

5.Before going to LVPL eye instituates, contacted mediassist customer care over toll free no.

6.Checked by mediassist customer care and told that the hospital is not in the list and told to go Kolkata nurshing home. Case was so critical could not procced and assured by mediassist to carry necessary treatment on reembursement basis

7.Eye specialist throughly exhaminied the eye and suggested to go for STBRVO

8. Necessary ECG/cardiogram done at Adityacare Hospital for the injection.

9.Friday was given the time by LVPL BBSR and necessary surgery done and released by hospital.

So where is our decission to carry out treatment at OPD.

Sympatheticaaly consider my case and approve the claim.

Pl donn't perceve and go by facts.
