PF refund

Dear Sir,

My name is Betsy Mary Thomas worked with Wockhardt Hospital, Cunningham, Bangalore. I joined there on 10th May. During my service, I have been transferred to Wockhardts Bannerghatta from where I resigned on 16th May.

When I had applied for my Provident Fund refund in the second time (there was no response for the first application) in the year the application had returned. The reason mentioned in the application was “I am not eligible for the scheme since my date of Joining and leaving did not coincide with actual date”.

As per records, there are two A/c. No. KN/14541/05098 and KN/14541/01056.

Kindly advice me how can I get my refund.
