Hi!

I have a health insurance policy in my wife name with APOLO MUNICH and following are the details--

I had submitted all the required documents to FHPL gurgaon phase IV office on date 13-Oct by hand personally for the reimbursement of medical expenditure.further some more documents were asked by you And I have submitted those required documents also on date 24-Oct. But till date I have not received any reimbursement of medical expenditure and further your Doctor also asked some statements where I have mentioned admission time & discharge time wrongly as that time I was not remembered however admission & discharge time was 8:00 AM & 8:00 PM respectively.
