Cast didn't dispense

On Dated 15/10/09 at 2:45 am I was about to widral RS 1000/-. I insert my card, enter secret code then widral RS 1000/- but cash didn’t dispense most probably because of machine fault. Then a window came with message that if you don’t collect within 30 seconds cash will be return back. I check my A/C balance again but the amount RS 1000/- had been deducted from A/C.

The ATM # is S1CW3497, ICICI Bank, Address is Presidency Bldg, Sector-14, Gurgaon, Haryana, India

So I need your kind attention to take care about this matter and add deducted RS 1000/- in my A/C ASAP.
