Complain against non receiving of the Health Insurance Policy

Dear Sir
This is to inform u that I applied for a Health Card for my mother on 12/04, but till date I have not received any insurance document or the Health Card or even the policy no. I am providing u the following details:

4) Receipt No-1003-00027840
5) Receipt Date-15/04
6) Cheque No-886632
7) Bank Name-ICICI Bank
8) Amount-6, 276
9) Salesperson Name-Arun Kumar Rai, Emp Code-05408 (Nehru Place, New Delhi)
10) Address: H.O-81. C/O-Surender Singh. Madangir. New Delhi-110062
My cheque has been encased on 16/04 but I have neither got any call nor got any document. I have submitted the relevant document, i.e- VodarID Card and Class 10 Certificate along with the form and cheque. The sales person who has done my policy is not picking up his phone, whenever I tried to contract him he either disconnect my call or transfer it to someone else. I have already called the customercare executive and she told me that my policy is pending because the healt checkup has not been done, but at the time of doing the insurance the sales person i.e. Arun Kumar Rai, Emp Code-05408 informed me that as my mother's age is 49 I need not do any medical checkup. So my question is that how a authorized sales person can provide wrong information and if he has mislead some customer why he is denying his responsibility and the second question is how a company can encased the cheque before the policy is approved. I called several times to the Nehru Place branch, New Delhi and asked for the Branch Manager no but nobody is entertaining me.
Therefore I request you to take necessary action against such offence.
