Denied cashless facility

I broke my meniscus in the knee while playing football last week, so my doctor told me that i need arthoscopy for that. then i taught of cashless surgery. after filling their pre-authorization form with all necessary documents attached the hospital personnel sent them to the concerned department for cashless treatment for approval. but they denied my treatment by saying that documents attached and i know that they are attached but they are Harassing me. if anyone can help and give a way then please tell me.
