Non receipt of Healthcard

Sir / Madam,

I have taken ICICI lombard Insurance health card for my family in 2008. Policy no.4034/CHT/03890986/00/000. Then I opted for a renewal of insurance in 2009. The Lombard People are debiting annual premium of rs.6326 in EMI for 12 months. But till date I have not recd the renewal healthcard for my family. So far None of the executives called me regarding renewal card issue. If the issue is not solved immediately I am prepared to cancel my renewal card. kindly take necessary action immediately or you will lose a customer.
