I state that my self Rajan Adlakha has Four Policies in your Company. Augusts my Policy no 243470721 This is to inform you that in the month of Jun I felt some pain in my stomach which was diagnosed due to kidney problem I intimidated this to you before I was attempted to R.G. Stone Delhi for treatment as suggested by Doctors as it was emergency so Doctors operated on me without waiting for the visit of your representative

For the Claim document was submitted to your office in the month July since then I am regularly following with your office but could not get any favorable response. As I have taken lone from my friends & relative for a very short period this amount has to be return to them as soon as my claim is pass.

In the light of above facts I once again request you to consider my claim favorably and don’t push me to contact the consumer court.
