I Lost my Nokia Cell phone No. 1650 black. I Have purchased this Phone at Univercell-Visakhapatnam on 12th January vide your Invoice No. P.VIS/771. Sorry to inform you that the name of the customer is RSM. DEEPAK, but you have typed in the Invoice is RSM. DEEOAK.

I have submitted all the relevent documents required for insurance, but sorry to inform you all the documents returned back simply because YOU HAVE NOT ISSED THE ORIGINAL COPY OF BILL. YOU HAVE ISSUED CARBON COPY OF BILL, THE NAME OF CUSTOMER IS WRONGLY TYPED.
